Report an absence to the BHS Attendance Team
If your student will be absent from school, please complete this form to notify the BHS Attendance Team. Providing this information helps us maintain accurate attendance records and ensure your child's safety and well-being.
Please complete all required fields and include the reason for the absence. If applicable, you may be asked to provide supporting documentation (such as a doctor's note or court documentation) in accordance with NYC Public Schools Attendance Policy.
For our full policy, please see the Attendance Policy & Guidance page.
New, lost, damaged, or non-working cards
Use this form to request an OMNY card if you are a new student who has not yet received one, or if your previous card has been lost, damaged, or is no longer working.
Please complete the form with accurate information to help us process your request as quickly as possible. Requests are typically processed within 24–48 hours, depending on the availability of replacement cards and current inventory. Once your request has been processed, you will be notified when your new or replacement OMNY card is ready for pickup or distribution.
If you have any questions regarding your OMNY card request, please contact the school's main office for assistance.
New students & replacement cards
Use this form to request a Student ID card if you are a new student who has not yet received one, or if your previous ID card has been lost, damaged, or is no longer usable.
Please complete the form with accurate information to help us process your request as quickly as possible. Requests are typically processed within 24–48 hours, depending on the availability of supplies and current demand. Once your request has been processed, you will be notified when your new or replacement Student ID card is ready for pickup.
If you have any questions regarding your Student ID card request, please contact the school's main office for assistance.
Device access for academic purposes
Use this form to request a school-issued laptop if your student needs access to a device for academic purposes.
Laptop requests are reviewed based on both student need and available inventory. Because the number of devices is limited, priority is given to students who do not have access to a computer or laptop at home. Students who already have access to a personal device may receive a laptop if additional devices are available.
After submitting the form, please instruct your student to visit Ms. Jackie during their lunch period to discuss their request and complete the laptop checkout process, if a device is available.
Students who are issued a school laptop may keep the device for the duration of their enrollment at BHS. Students are responsible for taking proper care of the laptop by keeping it clean, handling it responsibly, and ensuring it remains in good working condition. Any damage, loss, or technical issues should be reported to the school as soon as possible.
If a student withdraws, transfers, or is no longer enrolled at BHS, they are required to return both the school-issued laptop and its charger before leaving the school. Failure to return school-issued equipment may result in additional follow-up by the school.
We appreciate your patience and understanding as we work to provide technology resources to students based on need and available supplies.
55 minutes per week, reserved for families

If you would like to schedule a conversation with a teacher during their Parent Outreach Time, please contact them directly by email to arrange a convenient meeting time.
We encourage all parents/guardians to get involved with New York City public schools. Families are our closest partners in helping our 1.1 million students to thrive. Here are some ways you can connect with your child’s school.
Families of BHS are no longer using Konstella for school communications. Please take a moment to join other parents, in your student’s grade Whatapp group chat.
There is no volunteer or financial obligation required. Please click the corresponding link below to join. Thanks!
